Case Management services are
designed to provide short term Community Advocacy and long
term Case Management assistance to help consumers in their
community settings to lead lives of independence and choice.
Services are guided by a belief in the potential for recovery
of those who seek our help. Assistance is provided to clients
as they return to the community following hospitalization,
and support and discharge planning is provided to them while
they are in the hospital. We help all clients determine and
learn what is needed to successfully manage symptoms, and
to obtain entitlements, services, and supports. We link consumers
to all available and relevant services and we help them to
advocate for themselves whenever it is necessary and possible.
The majority of the services are provided with and by clients
in their own homes or community settings. The Case Management
program is staffed with a Program Supervisor, Hospital Liaison,
Clinical Case Manager, Case Manager Assessment Specialist,
and Case Managers, including Polish and Spanish speaking staff.
