As a program of the Adult Services Division
of Community Mental Health Affiliates, Inc. (CMHA), ACTT exists
for the express purpose of providing outreach services to
persons 18 years or older, who experience behavioral health
problems. Clinical, medical, life management and socialization
services are provided in an effort to assist the individuals
we serve to acquire the skills necessary to successfully participate
in community living while achieving optimal quality of life.
Services are provided directly by CMHA, in collaboration with
other CMHA services, other behavioral health providers in
the community, the consumer’s natural support systems
and with the consumers themselves. ACTT was designed to provide
clinical and intensive case management services for people
who experience behavioral health problems and those with behavioral
health and concurrent substance abuse problems, and who need
more support than traditional outpatient care and case management
can provide. Medication supervision services are available
on a daily basis. Staff of the ACTT Team include a Program
Coordinator, Clinical Case Managers, Intensive Case Managers,
Advanced Practice Nurses, Registered Nurses, and Psychiatrists.
The ACTT Team is available on site Monday through Friday;
8 AM to 9 PM; and on Saturday and Sunday from 8 AM to 12 Noon.
Staff are available on call at all other hours.
