Young Adult Services Assertive Community Treatment Team (YASACTT)

The YASACTT program is an extension of CMHA’s Adult Assertive Community Treatment Team, which has served adults with serious behavioral health and substance abuse issues in New Britain for over 10 years. The YASACTT program provides Intensive Case Management, Medication Management and Supervision, and Clinical Services to young adults aged 18 to 25. This program is able to serve up to 30 young adults with very serious mental health issues and/or serious substance abuse. Most of the young people face multiple challenges and have suffered serious trauma in their lives. Some of the clients served by this program will be young adults stepping down from the Residential services offered by the Transitional Youth Program (TYP). Others might be referred by DCF, and still others from family or community.

The young adults may be living in a Scattered Site Apartment of the TYP program; or with families, or independently. In all cases however, at the time of referral to the program, the person will be having significant difficulties managing the responsibilities of daily living in their current setting, and would be in need of daily monitoring and services to avoid more restrictive placement, and/or maintain and improve their functional level. Services are mobile and occur predominantly in the community. Crisis services are available.

The program is staffed with a Program Coordinator, 2.5 Case Managers, 2 Clinical Case Managers, an APRN and a Part Time Nurse. There is consultation available to staff and clients from the Vocational rehabilitation Counselor at TYP. The Team Time Social Club also has separate hours and staffing available to meet the unique social needs of the young adult population. The program’s goal is the recovery of the young adult. This guides the program’s efforts at developing life skills, rehabilitation, normalized programming and activities, and coordination with educational and vocational resources in the mainstream community. Whenever possible, the family of the young adult is involved in services and planning.
