Intensive Assertive Community Treatment Team (IACTT) 235 Beaver Street, New Britain, CT 06051 (860) 224-2044 IACTT is a 12-bed supervised, congregate site apartment program. Staff support is provided in the agency-owned apartment building on a 24 hour a day, 7 day a week basis. The IACTT program consists of 3 two-bedroom apartments, 3 single bedroom apartments and 3 efficiencies. IACTT utilizes on-site intensive case management services to support individuals in areas of daily living, including money management, personal care, cooking, housekeeping, and socialization skills. Residents learn effective ways to cope with their feelings and manage their symptoms while in a supportive environment. Eventually, as residents gain skills, develop supports, have increased self-confidence, and are able to use resources in the community, they work with staff to transition to a living situation that offers even more independence. Residents typically remain in the program for two years. Choices Residential Program 111 Clinic Drive, New Britain, CT 06051 (860) 224-8880 Choices Residential Program works specifically with young adults aged 18 to 24 who are transitioning from services provided by the Department of Children and Families to those of the Department of Mental Health and Addiction Services. There are eight congregate apartments in one building of a large apartment complex for those initially entering the program. As success is achieved and less supervision and support is required, residents move to one of four scattered apartments located within the same large apartment complex. Finally, youth transition to CMHA’s Case Management or ACTT Services. The young people served by this program have not lived independently and are faced with the multiple challenges all adolescents encounter as they enter the world of adult expectations and responsibilities. In addition, these young adults have experienced significant emotional and or substance abuse problems, and have often been institutionalized for much of their adolescence. Staff works with residents to achieve educational and vocational goals and develop daily living skills. Transitional Living Center (TLC) 33 Highland Street, New Britain, CT 06052 (860) 229-4840 TLC is a 17 bed residential program serving the needs of individuals recently discharged from a psychiatric institution and who, without the benefit of the program, would eventually require hospitalization. TLC provides residents the opportunity to gain the skills and experience necessary to live more independently in the community. Residential counselors assist clients in arranging appointments and entitlements and developing daily living, money management, personal care, cooking, housekeeping, and socialization skills. Eventually, as residents gain skills, develop supports, have increased self-confidence and are able to use resources in the community, they work with staff to transition to a more independent living situation. Residents actively participate in the upkeep of the facility and learn to adjust to increasing degrees of personal responsibility while concurrently developing constructive social relationships with other residents. All residents participate in service planning activities which focus on areas of symptom management, recovery from substance abuse and mental illness, employment issues, constructive use of community educational and recreational facilities, and methods of arranging and maintaining housing upon discharge from the Center. Harvest House 5 Hart Street, New Britain, CT 06052 (860) 229-4850 Harvest House is an 8 bed residential program designed for adult clients with severe and persistent mental illness who require a longer term, structured, home-like environment. Individuals establish skills needed for increasingly more independent living in a supportive and structured environment. Harvest House also helps ease the transition for individuals re-entering the community after extended hospitalizations. Through the use of client-centered goal planning, Activities of Daily Living (ADL) training, group therapy, individual sessions, medication monitoring and education, Harvest House creates opportunities for people to grow and become more independent. Program services are coordinated with the residents and their clinical and other community support service providers to ensure comprehensive, recovery-focused interventions. Parkview II at the Highlands 33 Highland Street, New Britain, CT 06052 (860) 223-1922 This 15-bed, intensively-supervised program is designed for individuals who have previously lived in state hospitals and have been prevented from community living by medical or severe behavioral challenges. The program provides a high level of supervision, intervention and support, and offers monitoring of medication and medical/dental conditions; socialization through individual and group activities; counseling; recreational activities, assistance with the management of finances; and linkages to appropriate community service providers. Parkview also assists residents in moving on to more independent living and working environments in the community of their choosing. Length of stay varies by individual need and desire, but averages approximately 18 months. |
