MyChart is a patient portal that gives you access to your health record in a secure online environment. It enables you to manage many aspects of your health care on demand.  

You need an activation code to sign up for MyChart, which is accessible through the following ways: 

• Request at your next appointment 

• Request online

• Request through contacting us at (860) 224-8192

Yes, you can request one online or at your next appointment. Please note that activation codes expire after 60 days. 

Your provider determines which type of results or notes might require an in person conversation. Therefore, test results or notes of very sensitive nature are not released to your MyChart account.

You need access to a computer or mobile device connected to the Internet and an up-to-date browser (such as Edge, Chrome, Firefox, or Safari). You can also use the MyChart mobile app on a mobile device. 

You can fill out the addendum form (WIP) to amend inaccurate information in your record.  

You can only direct message a provider if the message is initiated by the provider.  

Yes, you can view a family member's record through Proxy access. It allows a parent, guardian or conservator to log into their personal MyChart account, and then connect to information regarding their family member. You can request proxy access by contacting us at (860) 224-8192 or at you next appointment. 

You can deactivate your MyChart account in Account settings in the MyChart app or by contacting us at (860) 224-8192.

You may cancel your appointment through the MyChart account but will not be able to reschedule. Rescheduling requests are handled by our Call Center staff, who are available at (860) 224-8192. 

Yes, you can pay your bill under billing summary in MyChart.

Yes, you can request a formal copy of your record documented in EPIC under sharing hub in MyChart. For any records outside of EPIC, please submit a request by contacting the Medical Records department at (860) 348-2310.
